Forms

Please complete the appropriate New Patient forms online or manually.  If filling online, the completed forms must be opened with a PDF reader program such as Adobe Reader and can be emailed completed to Info(at)ZenFertility.com.  Otherwise, bring your completed forms to your initial visit.  If you are unable to fill out the forms prior to coming, arrive at least 15 minutes before your scheduled appointment time.

• Medical Records Release - Bringing a copy of your latest blood work and all semen analyses (if applicable) will provide us with the most accurate information so we can give you the best prognosis.  If you do not have a copy, fill out the medical records release form and give to your doctor so copies can be faxed to us.
